How Insurance Works

  • Membership visits are not billed to insurance.
  • Patients who carry insurance may still use it for services outside the primary care relationship, including laboratory testing, imaging, prescriptions, and specialist care.
  • Preferred pricing for labs and imaging may be available through select partners.
    All testing is coordinated with transparent, upfront pricing—simple and predictable..
  • Insurance-track patients do not have access to membership benefits.

Enrollment

Membership begins with a one-time $99 enrollment fee- currently waived.

This supports the clinical onboarding process, including medical record review, intake assessment, and personalized care planning, so your care begins with a clear understanding of your health history and goals.

Enrollment also covers:

  • Secure patient portal and messaging setup
  • Preventive care and screening review
  • Care coordination preparation with previous providers, pharmacies, labs, and specialists

Additional Services

Home visits (within 30 miles)

$300 per visit

Expanded advocacy services

$150 initial consultation fee; $75 hourly rate for continued care

Specialized Geriatric Care

$400 initial consultation fee; $150 hourly rate for continued care

Services may include:

  • Medication reconciliation and polypharmacy review
  • Preventive care and screening review
  • Post-hospital follow-up and care transitions
  • Cognitive screening when appropriate & Dementia management
  • Long-term care planning discussions

Medication Optimization Review with Clinical Pharmacist

$250–$350 per consultation

A comprehensive polypharmacy review includes:

  • Full medication reconciliation
  • Drug–drug interaction screening
  • Deprescribing recommendations
  • Renal and hepatic dosing adjustments
  • Beers Criteria screening for geriatric safety
  • Written clinical report
  • Optional coordination call

This consultation focuses on medication safety and clinical risk reduction
