Each visit is billed individually through insurance and is subject to deductibles, copays, and plan benefits.

Membership benefits are available exclusively through the membership care model.

We currently accept select UnitedHealthcare, Blue Cross Blue Shield of NC, and Aetna plans.

Coming Soon: Cigna and Medicare (pending approval).

$175 per visit

$150 initial consultation fee; $75 hourly rate for continued care

  • Preventive care and screening review
  • Post-hospital follow-up and care transitions
  • Cognitive screening when appropriate & Dementia management
  • Long-term care planning discussions

Medication Optimization Review with Clinical Pharmacist

$250 per consultation

A comprehensive polypharmacy review includes:

  • Full medication reconciliation
  • Drug–drug interaction screening
  • Deprescribing recommendations
  • Renal and hepatic dosing adjustments
  • Beers Criteria screening for geriatric safety
  • Written clinical report
  • Optional coordination call
